If you HAVE completed the electronic registration on the portal or are a returning patient, please arrive 15 minutes early with your x rays, medication list, insurance information, and form of payment.

If you have NOT completed the electronic portal registration, using the link to the right, please arrive 30 minutes early to your new patient appointment to complete these forms. Please bring your x-rays, a list of your medications, insurance information, and form of payment.
